Things to Bring

Below is a list of the things you will need to bring with you to your first appointment:


If you opt to use our Patient Portal, once registered, our staff will provide you with a welcome letter that contains your temporary password. Once you have access to your Patient Portal you will be able to complete your registration and medical history electronically prior to your appointment. Othwerwise, you will be required to complete the below forms and bring them to your appointment.

 - Registration Form 

 - Medical History Form

 - Notice of Privacy Policy

 - Financial Policy

 - Names and doses or labels of all medicines

 - Test results that might be pertinent to your care. If you have x-rays or other information from
   a different healthcare provider, please contact that provider to put your x-rays, scans or results
   on a CD-Rom disk.

 - Referral from your primary care provider's office (if required)

 - Photo Identification

 - All Active and Valid Insurance Cards
   Co-payment, co-insurance, and/or unmet deductible amount

If any of the above items is missing or incomplete, your appointment may be rescheduled for another day.

Please be prepared to give a urine sample upon arrival.

If you cannot keep your appointment, please call 48 hours in advance. Our team will remind you of your appointment prior to the day of your visit.

MISSED APPOINTMENTS: In fairness to other patients and to the doctor, we require at least 24 hours notice to cancel appointments. You may be charged a $25 fee for missed appointments.

Your doctor will thoroughly discuss the details of your medical care. If your doctor needs to order tests or surgery for you please provide our staff with your availability and allow a few days for the scheduling process to be completed. Our staff will then contact you within a week to follow up.
