As a courtesy, we will submit your insurance claim for you if the information is correct and accurate on the day of the visit. Please notify our office immediately if you experience any changes in your insurance plan, coverage, and/or benefits so that we can ensure our records are up to date. Arizona Institute of Urology is a participating provider for Medicare, AHCCCS and most major insurance plans. For a current list of accepted insurances click here. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
Your insurance company may require a co-payment or a coinsurance at the time of your appointment. You are responsible for charges that are not covered by your insurance plan. When necessary, our staff will work with patients to set up a payment plan. Most of the services we provide are covered by insurance; however, some individual contracts may have exclusions to particular procedures or conditions. Such exclusions and non-covered services, which may be deemed necessary by your physician, are the direct responsibility of the patient.
Our goal is to make the billing and payment process as simple as possible for you.
